77-year-old male admitted to Tuckerman Rehabilitation and Healthcare Center from Anne Arundel Hospital presenting with severe weakness and confusion found to have diskitis, bacteremia due to staphylococcus aureus, and urinary retention.

Nursing Interventions

Maintain Safety
Monitor Labs – Weekly labs including BMP, CBC, CMP
Medication Management – Diuretic, Beta Blocker
IV Antibiotics – Treat Infection
Pain Management

Therapy

Upon admission, patient required maximum assistance with bed mobility, functional transfers, and was not able to ambulate due to pain. The team approach of care to include the support of our on-site Physiatrist who also serves as our Medical Director Dr. Ronaghain, allowed the patient to participate in occupational and physical therapy 5 times a week. He is now supervision with bed mobility, minimal assistance with functional transfers and has ambulated 50+ ft min A with a rolling walker.

Patient continues to make gains and work with therapy!
